Comments arrive here by a few routes: your blocked words list, a channel setting that holds either all comments or only the ones YouTube's systems flag as potentially inappropriate, and YouTube's own automated detection. They sit in the Held tab for up to sixty days, and you can approve, ignore or remove each one. The failure mode is neglect. Detection is imperfect and regularly pulls in perfectly ordinary comments, so a channel that never opens the tab is quietly withholding replies from its own regulars while looking unresponsive to everyone who wrote one.
